Evergreen Health Signature Care

An innovative, personalized health care experience.

Our Team

Dr. Kristen Knox and Dr. Steve Montague have been part of a thriving primary care practice for more than 20 years. In 2014, the doctors decided they wanted to try a different approach to primary care medicine, one where they could spend more time and develop meaningful relationships with their patients.

They changed to their practice to concierge medicine, where the smaller practice size meant they could offer longer appointments along with individualized care that included wellness planning, disease prevention, healthy lifestyle coaching and regular diagnostic testing. They would also be available to their patients 24/7 by phone, text or email.

Signature Care is a service of EvergreenHealth, which is nationally recognized for clinical excellence, patient safety and quality. Should our patients need specialty care, we’re able to refer them to the best of EvergreenHealth’s network of more than 800 physician specialists in more than 80 specialties.
